The dosage for pediatric patients weighing greater than or equal to 17 kg who can swallow an intact tablet, one Tablet (150, 200, 250 or 300 mg based on body weight) once daily taken orally with or without food. Patients with a creatinine clearance of 30-49 mL/min should only take one dose every 48 hours. Those with a creatinine clearance of 10-29 mL/min may take one 300 mg pill two times a week, while patients undergoing hemodialysis should be restricted to one pill every seven days or 12 hours after dialysis.

Missed Dose

If a dose of Tavin medicine is missed, take it as soon as you remember. Skip the missed dose if it is almost time for your next scheduled dose. Do not use extra medicine to make up for the missed dose.
